Understanding care needs
A care coordinator assesses physical, cognitive, and functional needs to help determine appropriate support. They help clarify whether home care, short-term care, or long-term care may be appropriate.

Support at home & in the community
This includes personal support workers, nursing, therapies, and other in-home supports. There are respite
care options, which offer temporary support to give caregivers a break, either at home or through day programs.
Short-Term Care Options
In some situations, temporary care can help during recovery or transitions. This may include short stays in long-term care homes for recovery, caregiver relief, or other short-term needs.​
